The book Alveolar Cavitations describes fatty-degenerative osteonecrosis of the jawbone (FDOK)—a condition previously underestimated—as chronic aseptic and ischemic changes in the jawbone marrow, in which functional bone tissue is replaced by degenerated fatty tissue.
These lesions, which are difficult to detect on X-rays, are not simply a case of “poor wound healing,” but represent a distinct pathology with a specific morphological and immunological end-state. Osteoimmunological dysregulation between the bone and immune systems leads to characteristic patterns of cytokine expression, with singular overexpression of RANTES/CCL5.
At the same time, the classic inflammatory markers TNF-α and IL-6 are significantly reduced. This constellation explains a “silent,” chronic inflammation without typical clinical Th1/Type 1 inflammatory signs and is discussed as a possible source of systemic immunological effects, for example in autoimmune, tumor, or neurodegenerative diseases.
The FDOK clearly distinguishes Alveolar Cavitations from drug-induced osteonecrosis of the jaw, classifies them under ICD-10 (M87.0/M87.2) and ICD-11 (FB81.0), and questions both blanket surgical interventions and biased rejections of this clinical picture. Modern, radiation-free imaging techniques such as transalveolar ultrasonography (TAU) are presented as a diagnostic adjunct and are becoming increasingly important in clinical practice.
